Job Overview

The Patient Navigator plays a pivotal role in ensuring seamless transitions between care settings for patients and family members undergoing diagnosis, treatment, and follow-up for specific clinical service programs. This position collaborates with multidisciplinary teams to develop, manage, and coordinate activities related to these programs, focusing on optimal patient outcomes.


Responsibilities:
  1. Acts as a resource to interdisciplinary teams, patients, and families throughout the episode of care, facilitating efficient and coordinated care to achieve positive patient outcomes.
  2. Provides education, advocacy, and support to address psychosocial needs, identifies barriers to care, and navigates patients and family members through the diagnosis, treatment, and follow-up process.
  3. Utilizes evaluative and outcomes data to assess the quality of care provided and identify areas for improvement, ensuring consistent clinical outcomes.
  4. Develops customized plans for each patient referred, including scheduling, supportive resources, and post-treatment resources.
  5. Participates in multidisciplinary team discussions to address patient needs and collaborate on case management efforts.
  6. Assists with evaluating, selecting, and designing patient education materials and providing education and outreach programs to patients, staff, and community members.
  7. Demonstrates effective fiscal management by containing costs while ensuring high-quality, cost-effective care is provided.
  8. Contributes to the development of program goals and objectives and assists with implementing and maintaining quality programs that promote best practice patient care outcomes.

Requirements:
  • Certificate or Diploma from an approved nursing education program
  • BLS Certification from American Heart Association
  • RN License from South Dakota Board of Nursing (required); Bachelor's degree in Nursing and 1+ years of experience in Case Management and Patient Care (preferred)

Estimated Salary: $90,000 - $120,000 per year.
