About this Role:

Lifeworks Services is seeking a dedicated Service Coordinator to provide case management for our clients and families through our Lifeworks Financial Management Services. As an approved Financial Management Service through the State of Minnesota, we help individuals enrolled in the Community First Services and Supports (CFSS) Program. This is a great opportunity for a case manager to empower people to use services in their own communities.

Key Responsibilities:
  • Provide case management for Lifeworks Financial Management Services clients and families
  • Work as part of a greater Self-Directed Services team
  • Manage caseload with autonomy and deliver detailed work, stay organized, and meet deadlines
  • Establish and cultivate strong relationships with customers, county social workers, and administrative staff
  • Collaborate with cross-functional teams and deliver training to empower others to follow procedures
Requirements:
  • Four-year degree in human services or related field with one year of direct care to individuals with disabilities or individuals of 65 years of age or older
  • OR Two-year degree in human services or related field with two years of direct care to individuals with disabilities or individuals of 65 years of age or older
  • Ability to understand and communicate written and verbal directions
  • Regular attendance, reliability, and punctuality are necessary to meet the needs of the business
  • Well-organized with excellent attention to detail
  • Aptitude to work independently with excellent time management skills
