About the Role

St. Luke's Health System is seeking a dedicated and compassionate Palliative Care Social Worker to join our team. As a Palliative Care Social Worker, you will play a vital role in providing emotional, social, and spiritual support to patients and their families.

Key Responsibilities
  • Assess the social and emotional needs of patients and their families
  • Develop and implement individualized care plans to address these needs
  • Provide counseling and support to patients and their families
  • Collaborate with the interdisciplinary team to ensure comprehensive care
  • Document patient progress and outcomes
Requirements
  • Masters degree in Social Work
  • Licensed Masters Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in Idaho
  • Experience in Palliative Care or Hospice preferred
