Job Title: RN Care Manager

Trinity Health is seeking a highly skilled RN Care Manager to join our team at Samaritan Hospital in Troy, NY.

Job Summary:

We are looking for a dedicated and compassionate RN Care Manager to work with our Trinity Health medical group patients who are admitted to Samaritan Hospital. This role will focus on patients with multiple chronic diseases, CHF, and COPD, providing disease education, identifying and mitigating social influencers of health, coordinating needed services, and collaborating with provider offices to ensure integrated care through the healthcare continuum and improved health outcomes.

Responsibilities:
  • Provide disease education and support to patients with chronic diseases
  • Identify and mitigate social influencers of health
  • Coordinate needed services and collaborate with provider offices
  • Work with patients to develop and implement care plans
  • Monitor patient progress and adjust care plans as needed
Requirements:
  • BSN required (NYS Registered Professional Nurse licensure)
  • AD will be grandfathered and must meet performance standards
  • Previous experience required in care management at least one of the following settings: disease management, home health care, hospital or physician office setting
  • Preferred Certification in Case Management (CCM)
Preferred Qualifications:
  • 5 years of RN experience in Care Management
  • Prior electronic medical record (EMR) experience
  • Previous project leadership experience
  • Strong community resource knowledge
  • General knowledge of insurance plans and clinical resource management
What We Offer:

Trinity Health offers a competitive salary range of $33.65-47.15, based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
