Nurse Care Coordinator

About the Role:
We are seeking a skilled and compassionate Registered Nurse to join our team at Steward Health Care. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to a positive and supportive work environment.
Key Responsibilities:
- Coordinate total nursing care for patients, ensuring their physical, emotional, and psychological needs are met.
- Participate in patient and family teaching, promoting health literacy and empowering patients to take an active role in their care.
- Provide leadership and guidance to ancillary nursing and other patient team personnel, maintaining standards for professional nursing practice.
- Document patient assessments, care plans, and outcomes in a timely and accurate manner, adhering to hospital policies and procedures.
- Develop and implement effective, comprehensive, and individualized plans of care, incorporating interdisciplinary approaches and evidence-based practices.
- Apply knowledge and skills necessary to provide care appropriate to the age of the patient served, including infant, pediatric, adolescent, adult, and geriatric populations.
- Integrate, delegate, and coordinate patient care, utilizing a collaborative and patient-centered approach.
- Administer medication and perform treatments, adhering to established policies and procedures.
- Stay up-to-date with ongoing development of work skills, participating in in-services, formal educational programs, and on-the-job training.
Requirements:
- Graduate of an accredited school of Nursing, BSN preferred.
- Current licensure as a Registered Nurse in the state of practice.
- ICU-ACLS, Emergency Department-PALS and ACLS, Operating Room - ACLS, and MORCAP- Certification for Addiction Nursing preferred.
