Physical Therapist

VNA Health Group is seeking a skilled Physical Therapist to join our team in Ridgefield Park, New Jersey. As a Home Health Physical Therapist, you will be responsible for providing high-quality physical therapy services to patients in their homes or alternate community settings.
Key Responsibilities- Evaluate and treat patients with physical therapy needs, developing and implementing individualized plans of action in collaboration with physicians.
- Assess and document changes in patient condition, updating care plans and outcomes accordingly.
- Monitor the quality and appropriateness of services provided, adhering to agency infection control and safety policies.
- Adhere to the organization's policy on absenteeism and appearance.
- Physical Therapy Licensure in New Jersey required.
- ~2-3 years of prior clinical experience and community-based experience preferred.
- Strong computer skills.
