VNA Health Group is seeking a skilled Physical Therapist to join our team in Ridgefield Park, New Jersey. As a Physical Therapist, you will be responsible for providing high-quality physical therapy services to patients in their homes or alternate community settings.

Key Responsibilities
  • Evaluate and assess patients for physical therapy needs and develop individualized plans of action in collaboration with physicians.
  • Document all changes in patient condition and update care plans accordingly.
  • Monitor quality and appropriateness of services provided.
  • Adhere to agency infection control and safety policies.
  • Adhere to the organization's policy regarding absenteeism and appearance.
Requirements
  • Physical Therapy Licensure in NJ required.
  • ~2-3 years of prior clinical experience and community-based experience preferred.
  • ~Strong computer skills.
