We are seeking a dedicated and compassionate Social Worker to join our team at Thom Child and Family Services. As an Early Intervention Social Worker, you will play a vital role in providing home and community-based services to infants and toddlers with developmental delays and their families.

Key Responsibilities:
  • Conduct multidisciplinary team assessments to identify client developmental status.
  • Provide direct child development services and activities tailored to the needs of children 0-3 and their families.
  • Develop and implement Individual Family Service Plans (IFSPs) with a focus on identifying goals, objectives, and strategies for the child's development.
  • Document progress, complete reports, and maintain accurate records as required.
  • Collaborate with families to provide individual and group interventions, promoting child development and family support.
Requirements:
  • Master's Degree in Social Work from an accredited institution or equivalent certification/licensure.
  • Experience working with infants, toddlers, and families is preferred.
  • Knowledge of family systems and early childhood development is essential.
  • Excellent communication, organizational, and documentation skills.
  • Ability to maintain confidentiality and adhere to professional boundaries.
  • Current driver's license and reliable transportation.
