Position Title: Targeted Case Manager

Position Summary: Emerald is in search of dedicated individuals who are passionate about assisting clients in recognizing their inherent strengths to overcome challenges. Targeted Case Management emphasizes the identification of strengths and capabilities to improve skill sets, expand social networks, and connect clients with valuable community resources. This role may involve facilitating access to housing, medical care, social services, educational opportunities, and other essential services. Clients may present with Substance Use Disorders (SUD), Severe Mental Illness (SMI), or Severe Emotional Disabilities (SED).

This role entails managing a caseload of up to 25 clients, which includes direct client interactions, collateral contacts, travel, documentation, planning, and resource development. The distribution of responsibilities is approximately 50% direct service, 25% documentation, 15% knowledge and skill enhancement, and 10% administrative tasks.

Key Responsibilities:

  • Accept newly assigned cases following diagnostic intake by Emerald providers.
  • Conduct comprehensive targeted case management assessments and periodic reassessments for all clients in the caseload.
  • Collaborate with clients to develop suitable treatment plans and ensure the delivery of appropriate services.
  • Provide ongoing case management that aligns with the client's needs, objectives, and goals as outlined in the treatment plan.
  • Revise treatment plans as necessary to address emerging issues and ensure client needs are met.
  • Engage in planning and service coordination activities to enhance service continuity and effectiveness for clients, including collaboration with community partners.
  • Maintain close communication with intake, medical, and counseling staff to keep them informed of any changes in client progress.
  • Prepare and maintain comprehensive client records, including treatment plans, case notes, and required assessments, documenting achievements and progress.
  • Demonstrate proficiency in interviewing and assessing clients to evaluate their needs and develop appropriate treatment plans.
  • Identify and connect clients with community resources that may benefit them.
  • Establish and maintain effective working relationships with internal staff and community partners to eliminate barriers to meeting client needs.
  • Ensure all documentation adheres to Kentucky Medicaid and accreditation standards.
  • Stay informed of and comply with state reporting requirements for dependency, abuse, and neglect.
  • Possess knowledge of coping strategies, de-escalation techniques, and methods to enhance client motivation.
  • Exhibit the ability to work independently and prioritize effectively.
  • Participate in weekly staff meetings.
  • Provide case management services for a maximum of 25 individuals concurrently.
  • Perform additional duties as assigned by the supervisor, consistent with the position and agency policies.

Qualifications:

  • Bachelor's degree in Social Work, Sociology, Psychology, or a related human services field.
  • Completion of the targeted case management training program within six months of hire and adherence to certification requirements per Kentucky Board regulations.
  • Strong communication skills, a solid understanding of technology, and community engagement abilities are essential, including the capacity to coordinate meetings.
  • Flexibility to work a schedule that accommodates client needs and support networks.
  • Access to reliable personal transportation.
