Job Summary

The Kentucky Department for Public Health is seeking a highly skilled and dedicated professional to join our team as a Health Access Nurturing Development Services (HANDS) Specialist. This role is responsible for reviewing clinic records and conducting parent visit assessments/surveys to determine the level of support needed by families in the community.

Key Responsibilities
  • Conduct parent visit assessments/surveys within 30 days of receiving prenatal or birth records to determine eligibility for early childhood education and family support services.
  • Develop and distribute informational materials that meet program guidelines.
  • Keep accurate logs of service visits, schedules, and charting.
  • Prepare reports and analyses set forth in HANDS program policy and guidelines.
  • Interpret laws, regulations, policies, and procedures to provide administrative assistance or guidance to departmental staff.
  • Review records and reports submitted by program requests or agency staff to ensure compliance with federal and state law regulations and policies.
Requirements
  • Bachelor's Degree in Social Work, Nursing, Early Childhood Education, or a related field.
  • One year of experience performing case management services.
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
  • Ability to travel to family homes or within the community setting.
Working Conditions

Incumbents in this classification will typically perform their primary job duties under the following conditions:

  • Work will require travel to family homes or within the community setting when not in the agency office.
Additional Requirements
  • Applicants and employees in this classification will be required to submit to a drug screening test and background check according to the agency's policies.
