Job Summary

UnitedHealth Group is seeking a skilled RN Care Coordinator to join our team in Tarrant, Ellis and Dallas Counties, TX. As a key member of our Integrated Care Model, you will play a vital role in ensuring our members receive the best possible care.

Key Responsibilities:

  • Assess, plan, and implement care strategies tailored to individual member needs
  • Identify and initiate referrals for member transition to community resources and provide assistance with social service programs
  • Manage care plans throughout the continuum of care as a single point of contact
  • Communicate with stakeholders to ensure quality coordinated care and services are provided
  • Advocate for members and families to ensure their needs and choices are represented and supported

Requirements:

  • Current, unrestricted RN license in the state of Texas
  • 2+ years of experience working in the community health setting or in a healthcare role
  • Intermediate level of computer proficiency and ability to use multiple web applications
  • Reliable transportation and ability to travel in the assigned region
  • Access to a designated quiet workspace with the ability to secure Protected Health Information (PHI)
