Clinical Care Coordinator

Amelia Rehabilitation and Healthcare Center is seeking a skilled Clinical Care Coordinator to join our team. As a key member of our care team, you will be responsible for developing and implementing comprehensive care plans for our residents. Your expertise in MDS process and knowledge of federal and state regulations will be invaluable in ensuring the highest quality of care.
Key Responsibilities:
- Develop and implement care plans that accurately address the needs of our residents.
- Coordinate and manage the written plan of care for each resident, ensuring that goals are met and needs are addressed.
- Assist nursing management with the coordination, management, and review of nurse's notes to determine if the care plan is being followed.
- Monitor resident status changes to ensure timely nursing or clinical team involvement.
- Perform regular audits of documentation to ensure accuracy and compliance with regulatory requirements.
Requirements:
- Current State License as a Registered Nurse (RN)
- C.P.R. Certified
Preferred Qualifications:
- One year experience as a Nurse in a long-term care setting
