About the Role

We are seeking a highly skilled and compassionate Advanced Practice Registered Nurse to join our team at Georgia Hospice Care. As a Hospice and Palliative Care Specialist, you will play a vital role in providing high-quality patient care and support to individuals and their families.

Key Responsibilities
  • Provide diagnosis, treatment, and care to patients under the supervision of a physician
  • Focus on providing palliative care to patients and their families
  • Assist families along their healthcare journeys with love and compassion
  • Collaborate with the healthcare team to develop and implement individualized care plans
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies
Requirements
  • Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing
  • 2 years of experience within the last 5 years in chronic disease management/long-term care
  • Hospice background preferred
  • Reliable transportation
  • Ability to sit, stand, bend, move intermittently and lift at least 25 lbs and bear the weight of an average adult effectively
