Job Title: Early Intervention Service Coordinator II

Job Summary:

We are seeking a highly motivated and detail-oriented Early Intervention Service Coordinator II to join our team at Rocky Mountain Human Services. As a key member of our Early Intervention team, you will play a critical role in supporting children with developmental disabilities and their families by coordinating and monitoring services, scheduling home visits, and developing individualized family service plans.

Key Responsibilities:

  • Schedule and conduct home visits to assess the needs of children with developmental disabilities and their families.
  • Develop and implement individualized family service plans (IFSPs) to meet the unique needs of each child and family.
  • Coordinate and monitor services provided by early intervention providers, including scheduling appointments and ensuring timely documentation.
  • Provide advocacy and support to families in accessing community resources and services.
  • Collaborate with multidisciplinary teams to ensure comprehensive support for children and families.

Requirements:

  • Bachelor's degree in Early Childhood Education or a related field.
  • Minimum of five years of experience working with children with developmental disabilities and their families.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license and reliable transportation.

Preferred Qualifications:

  • Knowledge of early intervention systems and policies at the local, state, and national levels.
  • Experience working with children ages zero to three.
  • Two years of case management experience working with people with developmental disabilities and their families.
