About the Role: We are seeking a highly skilled Case Manager to join our team at Resources for Human Development. This is a challenging and rewarding role that requires a strong understanding of community resources, excellent communication skills, and a commitment to providing exceptional support to individuals in need.

**Key Responsibilities:**
  • Team-Based Planning: Work as part of a multidisciplinary team to provide holistic services to program participants.
  • Direct Client Care: Provide community-based services to help participants achieve their recovery plan goals.
  • Administrative Functions: Complete assessments and documentation as required by RHD, funder, licensing/accreditation entities.

Requirements:
  • Bachelor's Degree: In Human Services or related field.
  • Experience: One year Case Management experience preferred; two years' general work experience providing support to individuals in behavioral health, Addiction Recovery, or IDD services required.
  • Skills: Intermediate computer skills and valid driver's license.
