Job Overview:


The role involves reviewing and analyzing incident reports and various data to pinpoint trends and opportunities for risk mitigation and performance enhancement across the organization.


Identifies outcomes that are preventable versus those that are not through safety event classification, prepares reports on risk and patient safety trends for the organization, and aids in the monitoring of action plans linked to cause analysis activities, including root cause analysis.

Provides support during the investigation of safety incidents, operational assistance for relevant programs, and other departmental initiatives.

Acts as a representative for the department on hospital-wide committees.


Assists in conducting cause analysis activities, including apparent cause analysis, root cause analysis, failure modes and effects analysis, and common cause analysis.

Facilitates communication regarding risk management and patient safety activities throughout the organization.


Serves as a mentor to colleagues in the Safety First Mission Zero program, promoting patient safety concepts and skills across the organization, and contributes to the creation of committee meeting minutes, reports, educational materials, dashboards, and data entry into relevant software systems used by the department.

Supports departmental operations in alignment with the overall hospital strategies, policies, and procedures.


This position collaborates with internal teams to integrate, assess, and implement the impact of information on the development of safe practices throughout the organization, striving for high reliability in healthcare.


Qualifications:
A Bachelor's degree is required, preferably in a healthcare-related field, or a Master's Degree with relevant experience.


At least 3 years of recent clinical experience in an acute care setting as a healthcare professional or an equivalent combination of education and experience is necessary.

Prior experience in risk management or quality assurance within the healthcare sector is preferred.

Knowledge of medico-legal matters and risk management/patient safety programs is advantageous. Excellent communication skills, both verbal and written, along with strong interpersonal and facilitation skills are essential.

Proficiency in computer applications such as Excel, Word, PowerPoint, and Outlook is required.


Ability to work independently and interact effectively with all levels of staff and physicians throughout the organization.

Demonstrated collaborative and multitasking abilities, along with proven analytical and problem-solving skills.

Capability to perform report writing, take meeting minutes, and update/change/make project plans.

Flexibility in work schedule is necessary.

This position requires onsite presence.

License/Certification/Registration Requirements:
CPHRM or CPPS certification is required or must be obtained within 2 years of hire date.
RN or another clinical licensure is preferred.
