As a vital member of our team at AT Home Care & Hospice, the Hospice Registered Nurse (RN) Care Coordinator plays a crucial role in delivering compassionate care to patients and their families. This position involves direct patient care and collaboration with a multidisciplinary team to enhance the overall care experience.
Key Responsibilities:- Develop and implement individualized patient care plans, adjusting as necessary to meet evolving needs.
- Conduct thorough health assessments to establish nursing diagnoses.
- Administer prescribed medications and treatments in accordance with the care plan.
- Provide guidance and support to patients and their families to address nursing and related needs.
- Engage in regular communication with physicians and participate in interdisciplinary meetings to address the comprehensive needs of patients and caregivers.
- Ensure adherence to all applicable state and federal regulations and standards.
- Graduate of an accredited nursing program with a valid nursing license in the state of practice.
- A minimum of 1 year of nursing experience is required; 3+ years is preferred.
- Experience in hospice care or a similar environment is advantageous.
- Familiarity with the principles of hospice care is essential.
