As a Hospice Care Aide, you will play a vital role in providing personalized care and support to patients in their homes. Our team is dedicated to delivering exceptional patient care and making a positive impact in the lives of those we serve.

Responsibilities
  • Provide personal care services to patients, including bathing, grooming, and dressing, as assigned.
  • Assist patients with daily living activities, such as meal preparation, feeding, and ambulation.
  • Monitor patients' vital signs, including temperature, pulse, and blood pressure, as ordered.
  • Report any changes in patients' condition to the Case Manager.
  • Assist patients with light laundry needs and keep their living areas clean and orderly.
  • Participate in quality assessment and performance improvement activities.
  • Respect patients' and families' personal needs and environments.


Qualifications
  • Meet the training requirements of the State.
  • Completed a minimum of seventy five hours of classroom and supervised practical training.
  • Minimum of six months experience in providing personal care.
  • At least eighteen years of age.
  • Ability to read and follow written instructions and document care given.
  • Understands hospice philosophy and is comfortable providing specialized care to the terminally ill.
  • Satisfactory references from previous employers.
  • Ability to work with little direct supervision.
  • Secure with issues of death and dying.
  • Possess and maintains current CPR certification.
  • Demonstrates tact, patience, and good personal hygiene.
  • Successfully completes a probationary period.
  • Strong oral and written communication skills.
  • Good organizational skills.
