VNS Health is seeking a skilled and experienced Registered Nurse to join our team as a Home Health Preceptor. As a Preceptor, you will play a vital role in supporting the onboarding of newly hired RNs and LPNs, providing guidance and mentorship to ensure a smooth transition into our home health care team.
Key Responsibilities- Act as a preceptor for newly hired field RNs and LPNs, providing individualized instruction and guidance during the onboarding period.
- Provide ongoing feedback to branch management regarding individual nurse's learning progress and performance.
- Use a variety of tools to identify, document, and address orientation, performance, learning progress, and ongoing needs.
- Conduct and facilitate preceptor team meetings to promote effective communication and decision-making.
- Support the knowledge and skills development of new RNs and LPNs, including documentation, case management, clinical skills, and patient-centered care.
- Make joint visits with field clinicians to observe and instruct in current and new techniques, procedures, and treatments.
- Provide nursing care in accordance with VNS Health policies, practices, procedures, and Standards of Nursing Practice.
- Act as a subject matter expert to staff for information concerning patient care.
- Conform to established work procedures in relation to regional and program workflow.
- Understand, interpret, and disseminate VNS Health reimbursement policy to staff.
- Participate in selected student programs to enhance professional knowledge and provide training and guidance to staff.
- Participate in selected activities to various community audiences in conjunction with clinical operations.
- Provide input and feedback for continuous improvement of the onboarding process for new RNs and LPNs and learning and education needs for incumbent staff.
- Assume responsibility for personal professional growth through attendance at in-services, internal and external training, workshops, conferences, and continued formal education.
- A license and current registration to practice as a Registered Professional Nurse in New York State.
- Valid driver's license.
- OASIS certification preferred.
- Certification in Hospice and Palliative Care Nursing preferred.
- Four years' clinical home care experience required.
- Minimum of two years full-time clinical home care experience required.
- Minimum of one year of experience as a field staff nurse in VNS Health Home Care.
