About the Role

VNA Health Group is seeking a skilled Home Health Physical Therapist to join our team of dedicated healthcare professionals.

Key Responsibilities
  • Conduct thorough physical therapy evaluations and assessments to identify patient needs and develop personalized treatment plans.
  • Provide high-quality, patient-centered care in the comfort of patients' homes or community settings.
  • Collaborate with physicians and other healthcare professionals to ensure seamless care coordination.
  • Document patient progress and outcomes accurately and efficiently.
  • Stay up-to-date with industry developments and best practices through ongoing education and training.
Requirements
  • Physical Therapy Licensure in NJ required.
  • 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong computer skills and ability to navigate electronic health records.
  • Excellent communication and interpersonal skills.
  • Valid NJ driver's license.
