Job Title: Home Health Physical Therapist

VNA Health Group is seeking a skilled Physical Therapist to join our team in providing high-quality care to patients in their homes.

Job Summary:

We are looking for a compassionate and dedicated Physical Therapist to work with our patients in their homes, providing personalized care and support to help them achieve their goals.

Responsibilities:
  • Conduct thorough assessments and evaluations of patients to identify their physical therapy needs.
  • Develop and implement individualized plans of care to address patients' physical therapy needs.
  • Provide skilled physical therapy interventions to patients in their homes, including exercises, modalities, and education.
  • Collaborate with patients, families, and healthcare teams to ensure comprehensive care and support.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements:
  • Physical Therapy licensure in NJ required.
  • 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong computer skills and ability to use electronic medical records.
  • Excellent communication and interpersonal skills.
  • Valid NJ driver's license.
