Job Summary:

As a Hospice PRN RN with Concho Hearts Hospice, you will provide compassionate care to patients and their families, administering nursing care in their place of residence and coordinating care with the interdisciplinary group, patient, family, and referring agency.

Responsibilities:
  • Develop and Adjust Care Plans: Prepare and adjust patient care plans as needed, taking into account individual needs and preferences.
  • Support and Educate: Provide loved ones and primary caregivers with patient care information, end-of-life education, and emotional support, helping them navigate the hospice care journey.
  • Maintain Medical Records: Maintain and update medical charts, patient files, and health records, ensuring accurate and timely documentation.
Qualifications:
  • Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Licensure: Current license as a registered nurse (RN) and/or by the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC), current Drivers License.
  • Experience: Two years experience as a Registered Nurse in a clinical care setting, with hospice experience preferred.
  • Skills: Nursing skills as defined by generally accepted standards of practice, good interpersonal skills, and specialized skills in palliation and end-of-life care.
  • Transportation: Reliable transportation, valid and current auto liability insurance.
