We are a locally owned and operated hospice dedicated to providing compassionate care to patients in the comfort of their own homes. Our team of experienced healthcare professionals, including nurses, aides, social workers, chaplains, and volunteers, work together to provide symptom management, patient and family support, and spiritual care.
Job SummaryThe primary function of this role is to provide spiritual care to terminally ill patients and their families in the home setting. This involves assessing the patient's and family's spiritual needs and providing counseling to meet those needs. The ideal candidate will make reasonable efforts to facilitate visits by local clergy, pastoral counselors, or other individuals who can support the patient's spiritual needs. As a core member of the Interdisciplinary Group, you will participate in collaborative decision-making and contribute to the development of comprehensive care plans.
Key Responsibilities- Provide spiritual care to patients and their families in the home setting, addressing their emotional, social, and spiritual needs
- Assess patient and family spiritual needs and develop individualized care plans
- Maintain accurate and confidential records of patient interactions and progress
- Collaborate with other members of the healthcare team to ensure comprehensive care
- Foster relationships with local clergy, pastoral counselors, and other community resources to support patient spiritual needs
- Licensure: Per state guidelines, if any
- Experience: One (1) year experience as a Chaplain in a health care institution preferred
- Skills: Working knowledge of community spiritual and other resources; excellent interpersonal and communication skills
- Transportation: Reliable transportation, current auto liability insurance, and driver's license if personal vehicle is used for work
We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience. In addition to a comprehensive benefits package, including medical, dental, and vision insurance, paid time off, and professional development opportunities.
