Direct Support Professional

CareLink Community Support Services is seeking a dedicated and compassionate Direct Support Professional to join our team. As a Direct Support Professional, you will play a vital role in providing support and care to individuals with disabilities, helping them to live independently and achieve their goals.
Job Summary:
This position is responsible for providing direct support and care to individuals with disabilities, including assistance with daily living tasks, medication management, and transportation. The successful candidate will be a team player, able to work effectively with individuals, families, and other support staff to ensure the best possible outcomes for our consumers.
Key Responsibilities:
- Provide direct support and care to individuals with disabilities, including assistance with daily living tasks, medication management, and transportation.
- Assist with the development and implementation of individualized service plans, treatment activities, and case conferences.
- Monitor and report on consumer progress, identifying areas for improvement and developing strategies to address them.
- Collaborate with other support staff to ensure a comprehensive and coordinated approach to care.
- Participate in staff meetings and training programs to stay up-to-date on best practices and industry developments.
Requirements:
- High school diploma or equivalent required; bachelor's degree preferred.
- Valid driver's license and reliable transportation required.
- CPRP/RPRP certification preferred.
- Ability to work effectively in a fast-paced environment with frequent deadlines.
- Excellent communication and interpersonal skills.
- Ability to multitask and be flexible with regard to workload and assignments.
