Job Summary

The Service Coordinator/Home Visitor plays a vital role in providing comprehensive support to eligible families. As the single point of contact, this professional integrates direct services with existing program services, ensuring a seamless continuum of care.

Key Responsibilities
  • Conduct home visits with caseload children and families, utilizing a prescribed curriculum to promote positive outcomes.
  • Assess family needs, collaborate with agencies, and develop Individualized Family Plans to address specific requirements.
  • Update databases regularly, ensuring accurate and timely information.
  • Facilitate transitions to preschool services, ensuring a smooth handoff.
  • Collaborate with the Home Visit Coordinator for supervision, administrative and reflective.
  • Document work schedules, client visits, and related meetings in Outlook.
  • Provide home visits to 45-60 children, planning, assessing, and coordinating services.
  • Average time to first visit is within 14 days.
  • Paperwork is submitted within 3 working days.
  • Screenings, tools, and family plans are completed on time, with a documented family reason for any delays.
  • Time tracking is accurate and reflective of daily work.
  • Supplies are used efficiently.
  • Willingness to accept and process new families is demonstrated.
  • Discharge of clients who have met goals is expedited.
  • Assistance with new staff orientation is provided.
  • Promotion of a seamless continuum of care between all aspects of care and community resources is ensured.
  • Confidentiality and consumer relations policies are adhered to.
  • Efficient, orderly operations of the department are contributed to.
Requirements
  • Bachelor's degree in Social Work.
  • LSW licensure preferred.
  • 1-3 years of job-related experience, with a minimum of two years' experience working with young children and their families.
  • Valid driver's license and insured safe automobile.
  • Non-smoker.
