Position Overview:

This is a flexible role within Northwestern Memorial Healthcare, designed to provide essential support during staff shortages, including PTO and sick leave coverage.

Key Responsibilities:
  • Develop personalized care plans in collaboration with patients, their families, and the healthcare team.
  • Facilitate and lead care discussions for patients with extended hospital stays.
  • Document any unusual incidents and patient-related concerns, making necessary referrals to appropriate departments.
  • Evaluate patient and family needs for suitable discharge services.
  • Coordinate transitions to the appropriate level of care, including rehabilitation and home health services for complex cases.
  • Maintain up-to-date knowledge of community resources to ensure a continuum of care.
  • Deliver Medicare discharge appeal rights notifications in a timely manner.
  • Conduct medical record reviews to assess the appropriateness of admissions and ongoing hospital stays.
  • Monitor avoidable hospital days and recommend service or process improvements.
  • Ensure adherence to managed care agreements and length of stay objectives while upholding quality standards.
  • Educate staff on strategies to minimize length of stay and resource use while maintaining care quality.
  • Identify cases that require social work intervention and collaborate with healthcare professionals to enhance understanding of the social and emotional factors affecting patient health.
  • Advocate for patient rights and access to resources that enhance compliance and quality of life.
  • Identify and address psychosocial needs impacting adherence to medical recommendations through individual or group sessions.
  • Provide referrals and services aimed at helping patients achieve optimal social and health adjustments.
  • Utilize community resources to support patients in reintegrating into their communities.
  • Prepare comprehensive patient histories, service plans, and reports, ensuring accurate documentation of social work activities.
  • Offer emotional support and counseling to patients and families coping with illness, hospitalization, and financial challenges.
  • Report cases of abuse, neglect, and domestic violence to the appropriate authorities and guide mandated reporters.
  • Assist in transferring patients to suitable treatment facilities, including those for substance abuse and psychiatric care.
  • Engage families in discussions regarding organ and tissue donation and facilitate referrals.
  • Act as a liaison to the ethics committee, assisting patients and families with complex ethical decisions.
  • Provide crisis intervention for patients and families, responding to emergencies in various settings.
  • Assess patients with acute psychiatric needs and facilitate appropriate transfers.
  • Initiate goals of care conversations with at-risk patients.
  • Communicate care plans effectively with attending physicians and interdisciplinary teams.
  • Develop educational programs for patients and families on advance directives and related topics.
  • Participate in interdisciplinary team meetings and care conferences.
  • Fulfill all discharge planning responsibilities for patients in palliative care.
  • Serve as the primary social work resource for inpatient palliative care patients, coordinating care and providing emotional support.
