Job Summary

Key Responsibilities
  • Conduct thorough assessments and develop personalized care plans to ensure optimal patient outcomes
  • Collaborate with healthcare professionals, family members, and caregivers to ensure seamless care coordination
  • Provide education and support to patients and their families on disease management, medication adherence, and self-care techniques
  • Monitor patient progress, identify potential issues, and implement interventions to prevent complications
  • Communicate effectively with patients, families, and healthcare teams to ensure timely and accurate information exchange
Requirements
  • Current licensure as a Registered Nurse in the State of Minnesota
  • Minimum of two years of experience in medical-surgical nursing, with a focus on case management and patient education
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment
  • Ability to travel within a designated geographic area, with reliable transportation and a valid driver's license
Preferred Qualifications
  • Experience in community health and medical-surgical nursing
