Key Responsibilities:
  • Assess, plan, and implement care strategies tailored to individual needs and directed toward the most appropriate, least restrictive level of care.
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
  • Ensure care plans include covered services, EPSDT, in lieu of services, value-added benefits, informal supports, and address social determinants of health.
  • Communicate with stakeholders to ensure quality coordinated care and services are provided expeditiously to all individuals.
  • Advocate for individuals and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team.
  • Conduct home visits in coordination with individuals and care teams, which may include community service coordinators.
  • Serve as a resource for community care coordinators, if applicable.

Requirements:
  • Bachelor's Degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or a related human services area.
  • 1+ years of experience working with individuals with long-term care needs.
  • 1+ years of experience working within the community health setting in a healthcare role.
  • Intermediate level of computer proficiency, including Microsoft Outlook, Teams, and multiple web applications.
  • Ability to travel 75% of the time for the role.
  • Valid driver's license, current automobile insurance, and access to reliable transportation.
Preferred Qualifications:
  • Licensed Social Worker or clinical degree.
  • 1+ years of experience working with individuals receiving services on one of the home and community-based waivers in KS.
  • Background in managing populations with complex medical or behavioral needs.
  • Experience with electronic charting and arranging community resources.
Soft Skills:
  • Strong communication and customer service skills, both in person and via phone.
  • Ability to work independently and maintain good judgment and accountability.
  • Demonstrated ability to work well with others.
  • Strong organizational and time management skills.
  • Ability to multi-task and prioritize tasks to meet all deadlines.
  • Ability to work well under pressure in a fast-paced environment.
