Hospice Nurse

We are seeking a dedicated and skilled Registered Nurse to join our Rapidan team, providing compassionate care to individuals in need. As a Registered Nurse in our hospice program, you will play a vital role in ensuring the comfort, dignity, and well-being of our patients.
Responsibilities:- Perform nursing evaluation visits, develop plans of care in conjunction with physician's orders, and coordinate with interdisciplinary team members to ensure optimal comfort and quality of life.
- Assess and re-evaluate patient needs, consulting with physicians, reporting changes, and communicating with others when a change in the Plan of Care is necessitated.
- Provide education to patients, families, and caregivers about disease progression, symptom management, and end-of-life care options.
- Coordinate care with other healthcare professionals, including physicians, social workers, chaplains, and CNAs, to ensure a solid team approach to patient care.
- Maintain accurate and up-to-date documentation of patient assessments, care plans, and interventions.
- Participate in interdisciplinary team meetings, providing valuable input and contributing to the overall care plan.
- Current and unrestricted Registered Nurse (RN) license in the state of Virginia.
- Minimum of 2 years of nursing experience preferred, preferably in hospice, palliative care, or a related field.
- Knowledge of hospice regulations, practices, and standards of care with a demonstrated ability to manage all aspects of terminal care, health team service coordination, documentation, and case management.
- Excellent clinical assessment and critical thinking skills, emotionally mature with the ability to cope with family stressors and tolerance of individual lifestyles.
- Strong communication and interpersonal skills, with the ability to establish rapport with patients, families, and healthcare professionals.
- Compassionate and empathetic nature, with a genuine desire to provide comfort and support during challenging times.
- Ability to work independently and as part of a team, collaborating effectively with interdisciplinary professionals.
