Qualifications

Applicants must possess a current, full, active, and unrestricted Oregon Professional Registered Nurse license.

Additionally,

Applicants must have a valid driver's license and maintain an acceptable driving record.

Furthermore,

Experience in home health care, clinic settings, skilled facilities, or medical-surgical units, including comprehensive triage nursing, is required.

Responsibilities

The Stabilization and Crisis Unit (SACU) operates 20 residential group homes along the I-5 corridor, providing 24-hour care and supervision for individuals with intellectual/developmental disabilities, often accompanied by mental health diagnoses. Our mission is to assist individuals in safely transitioning back to their home communities through training and support. We emphasize positive attitudes while delivering health and medical support, psychological services, emotional and behavioral assistance, life skills training, educational support, recreational opportunities, and community integration.

As a Registered Nurse, your responsibilities will include ensuring that all nursing and medical needs of individuals are met, which encompasses:

  • Building relationships that maintain a holistic focus on physical, psychosocial, mental, emotional, social, and spiritual health.
  • Providing continuous and comprehensive preventive, curative, rehabilitative, and palliative care services.
  • Collaborating with a multidisciplinary team, including physicians, therapists, dieticians, pharmacists, and family members, to deliver comprehensive care.
  • Actively participating as a key member of the Individual Support Plan team.
  • Delivering direct nursing care to individuals with complex medical needs, which may include:
    • Respiratory management, including tracheostomy care and ventilator management.
    • Skin, ostomy, and wound care.
    • Enteral feeding administration.
    • Catheterization procedures.
    • Medication administration through various routes.
    • Assisting with activities of daily living (ADLs), including personal care and mobility support.
Working Conditions
  • A valid driver's license and an acceptable driving record are required.
  • Frequent lifting of up to 75 pounds for transferring and positioning individuals.
  • Regular squatting, bending, and stooping.
  • Exposure to infectious diseases and bodily fluids.
  • Daily contact with cleaning agents and detergents.
  • Varied work settings, both in residential homes and community environments.
  • Occasional exposure to inclement weather when accompanying clients to appointments.
  • Availability for overtime may be necessary to ensure adequate staffing and support.
  • Transportation of individuals using accessible vehicles may be required.
  • Maintaining a valid State of Oregon Registered Nurse License and current BLS certification is essential.
  • Frequent interaction with the public and other agency personnel.
  • Completion of biannual Bloodborne Pathogen Training is mandatory.
