Job Summary

Clinical Management Consultants is seeking a highly skilled Medical Social Worker Case Manager to join our team. As a key member of our interdisciplinary team, you will play a vital role in facilitating discharge planning and addressing the complex care needs of patients.

Key Responsibilities
  • Discharge Planning and Case Management: Collaborate closely with the interdisciplinary team to develop and implement effective discharge plans that meet the unique needs of each patient.
  • Mental Health Assessments and Interventions: Conduct comprehensive mental health assessments and provide evidence-based interventions to address the psychosocial needs of patients.
  • Crisis Intervention and Safety Planning: Develop and implement crisis intervention plans to ensure the well-being of patients, and provide safety planning to prevent harm.
  • Substance Use Disorder Assessment and Interventions: Assess patients for substance use disorders and provide evidence-based interventions, including SBIRT.
  • Complex Case Management: Identify and navigate available resources to address complex family dynamics, mental health issues, and other psychosocial complexities.
  • Interdisciplinary Collaboration: Work closely with Nurse Care Managers, physicians, nursing staff, and other relevant personnel to determine the appropriate involvement of social work in addressing patient care needs.
Qualifications
  • Education: A degree in Social Work or a related field from an accredited institution.
  • Licensure: Licensure as a Social Worker, as required by state regulations.
  • Experience: Experience in a healthcare setting, preferably working as a Social Work Case Manager.
  • Skills: In-depth knowledge of mental health assessment techniques and evidence-based interventions, familiarity with substance use disorder assessment and interventions, including SBIRT, strong crisis intervention skills, and proficiency in assessing and addressing complex psychosocial issues.
  • Personal Qualities: Excellent communication and collaboration skills, ethical awareness, and a commitment to providing compassionate and patient-centered care.
