Job Summary

The Long Term Care Social Worker is a vital member of our interdisciplinary team, responsible for providing psychosocial support and services to residents in our long-term care facility. This role requires a strong understanding of geriatric care and the ability to work collaboratively with residents, families, and healthcare professionals to ensure the highest quality of life for our residents.

Key Responsibilities
  • Conduct thorough social histories and psychosocial assessments to identify residents' needs and develop individualized care plans.
  • Collaborate with the interdisciplinary team to develop and implement care plans that address residents' physical, emotional, and social needs.
  • Provide counseling and support to residents and their families to help them cope with the transition to long-term care and address any concerns or issues that may arise.
  • Facilitate care plan meetings and ensure that residents and their families are informed and involved in the care planning process.
  • Develop and implement therapeutic interventions to address residents' social, emotional, and psychological needs.
  • Provide education and support to residents and their families to help them understand their care and services.
  • Act as a liaison between residents, families, and healthcare professionals to ensure seamless communication and coordination of care.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote resident-centered care.
Requirements
  • Bachelor's degree in Social Work, Human Services, or Psychology.
  • Minimum 1 year of experience in a healthcare setting, preferably in geriatrics.
  • Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals.
  • Ability to work collaboratively as part of an interdisciplinary team.
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
Preferred Qualifications
  • Master's degree in Social Work, Human Services, or Psychology.
  • Experience with MDS, discharge planning, and electronic medical records.
