We are seeking a dedicated RN Admissions Nurse to become part of our exceptional team. Our mission is to foster a supportive workplace that employs staff-friendly strategies and a collaborative approach, ensuring our team is well-trained and our residents receive exemplary care.

Key Responsibilities:

  • Conduct thorough admissions of new patients, including nursing assessments, while adhering to the operational standards of our facility.
  • Work collaboratively with the Director of Nursing and Administration to coordinate prospective admissions.
  • Ensure timely scheduling of follow-up consultations by the Unit Secretary.
  • Complete all necessary nursing assessments, including the initiation of the MOLST.
  • Maintain compliance with all state regulations and facility policies, procedures, and standards of conduct.
  • Assess, plan, implement, and evaluate individualized nursing care based on patient needs.
  • Document all data in the Electronic Medical Record and complete the Admission Nursing Note.
  • Conduct Fall Risk Assessments and develop intervention care plans as needed.
  • Perform Pressure Ulcer Risk Assessments using the Braden Scale and implement preventative measures for high-risk patients.
  • Establish Initial/Interim Care Plans addressing specific resident conditions.

Qualifications:

Long-term care experience is preferred, particularly in admissions.
