Medical Office LPN Primary Care Women's Wellness- Norwalk, CT

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Nuvance Health's Women's Wellness department in Norwalk, CT. As an LPN, you will play a vital role in providing high-quality patient care and supporting our team-based care initiative.
Key Responsibilities- Provide clinical support to ensure smooth daily department operations in assigned medical practices.
- Complete all visit documentation, including provider exams and proposed orders.
- Assure patient and examination area preparation.
- Complete clinical tasks under the direction of the provider or registered nurse.
- Follow up with pre-visit planning, focusing on care gaps, and lead team huddles with clinical staff.
- Perform intake activities, document patient information, and update patient records.
- Communicate test results, assist with prior authorizations and referrals, and propose medication renewals.
- Provide direct patient care, including vaccination and medication administration, and basic wound care.
- Manage tasks and messages in a timely fashion and schedule future appointments and referrals.
- Basic Life Support (BLS) certification within 30 days of starting.
- Licensed Practical Nurse licensure.
- Basic medical knowledge and terminology.
- Computer skills/data entry.
- Skilled in obtaining accurate patient vitals.
- Vaccine administration and medication preparation.
- Minimum one year of experience.
- Proficiency in multiple languages.
- Phlebotomy.
- EKG.
This role is based in Norwalk, CT, and requires a 40-hour workweek with 8-hour shifts, Monday to Friday, between 7:45am and 5:30pm.
