Job Summary

To provide high-quality nursing care to terminally ill patients in their homes, while also offering emotional support and guidance to their families. As a member of the Hospice team, you will work collaboratively with other healthcare professionals to deliver comprehensive care and ensure the best possible quality of life for our patients.

Key Responsibilities
  • Assess and reassess the nursing needs of patients, developing and implementing individualized care plans to meet their unique needs.
  • Provide education and support to patients and their families, empowering them to take an active role in their care.
  • Collaborate with the interdisciplinary team to coordinate care and ensure seamless transitions between healthcare settings.
  • Monitor patients' conditions, identifying and addressing any changes or concerns in a timely manner.
  • Document patient interactions, progress, and outcomes in accordance with regulatory requirements.
Requirements
  • Graduate of an accredited school of nursing with a current RN license in good standing.
  • Minimum 2 years of experience as a registered nurse, with a preference for those with hospice or palliative care experience.
  • Ability to work independently, make sound judgments, and prioritize tasks effectively.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare teams.
  • Valid driver's license and reliable transportation.
Working Conditions

This role requires the ability to work in a variety of settings, including patients' homes, hospitals, and other healthcare facilities. You must be able to tolerate exposure to elements such as odors, blood, and body fluids, as well as work in adverse environmental conditions.

Essential Functions
  • Regularly assesses and reassesses the nursing needs of patients.
  • Provides dietary counseling and education to patients and their families.
  • Initiates nursing procedures and treatments as needed, following physician orders.
  • Observes signs and symptoms, reporting any changes or concerns to the physician and IDG members.
  • Teaches, supervises, and counsels patients and their families on providing care for the patient.
