Key Responsibilities:
  • Conduct thorough patient assessments to identify needs and develop effective care plans
  • Evaluate patient progress and outcomes, making adjustments as needed
  • Collaborate with the healthcare team to coordinate care and ensure seamless transitions
  • Provide education and support to patients and families on treatment plans, procedures, and preventative measures
  • Participate in Continuous Quality and Performance Improvement initiatives to enhance patient care and outcomes
Requirements:
  • Current RN licensure in the state
  • Graduate of an accredited wound ostomy care nurse program with current licensure
  • At least 2 years of experience in wound care or a related specialty
  • Basic Cardiac Life Support certification; Advanced Cardiac Life Support certification preferred
  • Wound Ostomy Care Nurse (WOCN) certification preferred
