Responsibilities
  • Provide Direct Patient Care: Deliver in-home direct patient care according to the provider's prescribed plan of care, while closely monitoring progress and recovery.
  • Assess and Document Patient Conditions: Assess and document patient conditions and report changes as needed, including vital signs, blood/stool/urine, glucose, etc.
  • Administer Medication and Treatments: Administer medication, insulin, and IV/fluids, documenting thoroughly, and inspect and care for wounds, change dressings, and assist with personal hygiene.
  • Collaborate with Healthcare Professionals: Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes.
Requirements
  • Education and Experience: High school diploma or equivalent, Oasis experience, and a minimum of 12 months of experience as a Home Health Nurse (RN).
  • Communication and Interpersonal Skills: Strong communication and interpersonal skills.
  • Certifications and Background: Current BLS certification, two (2) positive work references, and successfully complete and pass a background check and drug test.
