Job Summary

Patriot Homecare is a rapidly growing, quality-focused, family-owned and operated local home health network seeking a PRN Home Health Aide to join our team.

We are looking for a compassionate and skilled individual to provide personal care and related services in the home, under the direction, instruction, and supervision of our staff nurse and Nurse Supervisor.

Key Responsibilities
  • Provide safe, competent care to clients, following the plan of care and adhering to established protocols.
  • Assist clients with maintaining good personal hygiene and creating a healthful, safe environment.
  • Support clients with ambulation, as ordered by the physician and approved and supervised by the nurse.
  • Collaborate with therapy personnel as needed to facilitate rehabilitative processes.
  • Encourage clients to become as independent as possible, according to the nursing care plan.
  • Report any changes in the client's mental or physical condition or in the home situation to the staff nurse.
  • Coordinate scheduling of visits to ensure seamless care delivery.
Requirements
  • High school diploma or equivalent preferred.
  • Valid STNA License required.
  • Completion of a minimum of 75 hours of training, including an introduction to personal care services.
  • Must be free from health problems that may be injurious to patients, self, and coworkers.
  • Current CPR license required.
  • Criminal background check required.
Work Environment

The ability to drive and access clients' homes, which may not be routinely wheelchair accessible, is required.

Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and provide client care.

Occasional bending, stooping, reaching, and moving client weight up to 250 pounds may be required.

This position is paid at a competitive PRN rate.
