Job Title: Home Health Speech Language Pathologist

Alternate Solutions Health Network LLC is seeking a skilled Home Health Speech Language Pathologist to join our team in Cuyahoga Falls, Ohio.

Job Summary:

We are looking for a compassionate and experienced Speech Language Pathologist to provide high-quality home-based care to our patients. As a Home Health Speech Language Pathologist, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that promote optimal patient outcomes.

Responsibilities:
  • Develop and implement individualized plans of care for patients with speech and language disorders.
  • Provide home-based speech therapy services to patients, including assessment, treatment, and education.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
  • Document patient progress and outcomes in accordance with agency policies and regulations.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current licensure as a Speech Language Pathologist in the state of Ohio.
  • Minimum one year of experience as a Speech Language Pathologist in a home health or acute care setting.
  • Valid driver's license and reliable transportation.
  • Ability to work independently and as part of a team.
