Care Coordinator

We are seeking a highly skilled Care Coordinator to join our team at Integrated Services for Behavioral Health (ISBH). As a Care Coordinator, you will play a vital role in providing exceptional care to children, youth, and families with significant behavioral health needs.
Job Summary:The Care Coordinator will be responsible for service linkage and care coordination, engaging and working with children, youth, and families to ensure they receive the necessary support and resources to achieve their goals.
Key Responsibilities:- Develop and implement individualized care plans that address the unique needs of each child, youth, and family.
- Collaborate with community agencies and resources to provide comprehensive support services.
- Engage and build positive relationships with children, youth, and families to ensure successful integration of care plans.
- Provide education and support to families on available resources and services.
- Ensure that services identified on care plans are the most appropriate, least restrictive, and meet the safety and treatment needs of the child, youth, and family.
- Participate in program evaluation and attend team and program planning meetings.
- Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field.
- Three years of experience with a high school diploma or equivalent; or two years with an associate degree or bachelor's degree; or one year with a master's degree or higher.
- Knowledge and experience in Hi-Fidelity Wraparound preferred.
- Two years of experience in a coordinated supportive services or care coordination role preferred.
- Experience working with people with autism spectrum disorders and developmental disabilities preferred.
- Competitive salary range: $20.19-$25.03 per hour based on experience, education, and/or licensure.
