Job Title: Care Coordinator

We are seeking a highly skilled Care Coordinator to join our team at Integrated Services for Behavioral Health. As a Care Coordinator, you will play a vital role in providing exceptional care to children, youth, and families with significant behavioral health needs.

Job Summary:

The Care Coordinator will work closely with families to identify care coordination needs and services, ensuring the best possible outcomes for children, youth, and families. This role requires a thorough understanding of local communities, strong relationships with community agencies, and the ability to identify potential community supports for development.

Key Responsibilities:

  • Collaborate with families to define care coordination needs and services, ensuring alignment with service delivery standards and program outcomes.
  • Work with families to identify strengths and cultural factors that influence their functioning and family interaction styles.
  • Coordinate family-based services for children, youth, and families in their home, school, and community.
  • Ensure that services identified on care plans are the most appropriate, least restrictive, and meet the safety and treatment needs of the child, youth, and family.
  • Develop and maintain positive relationships with children, youth, and families, engaging them in care planning and supporting healthy outcomes.
  • Build collaborative partnerships with community resources to meet the diverse needs of youth and families.
  • Maintain accurate documentation, participate in program evaluation, and attend team and program planning meetings.

Requirements:

  • Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field.
  • Three years of experience with a high school diploma or equivalent; or two years with an associate degree or bachelor's degree; or one year with a master's degree or higher.
  • Knowledge and experience in Hi-Fidelity Wraparound preferred.
  • Two years of experience in a coordinated supportive services or care coordination role preferred.
  • Experience working with people with autism spectrum disorders and developmental disabilities preferred.
