Job Title: Home Health Nurse Specialist

Overview
At Residential Home Health and Hospice, we believe in delivering exceptional care through a team-based approach. As a Home Health Nurse Specialist, you will lead and collaborate with our team to provide patient care plans that make a real difference.

Salary: $120,000 - $180,000 per year (including first year success bonus)

This role supports patients in Port Huron and surrounding areas. You will be responsible for providing management and delivery of patient care plans, working closely with our team to ensure seamless care transitions. Requirements
We are looking for compassionate nurses with:
  • RN license in the state you work
  • Graduate from an approved school of practical nursing
  • One year of RN experience and the clinical competence to deliver quality patient care
  • Current driver's license and ability to spend ~20% of your day driving to/from patient locations
  • A commitment to consistently meet critical deadlines for charting
  • The skills needed to self-manage your time and schedule
  • Demonstrated experience with tablets, mobile phones and EMR software
