Job Summary

Liberty Health is seeking a compassionate and skilled Hospice Aide to provide high-quality patient care and support to individuals facing terminal illness.

Key Responsibilities:

  • Deliver direct patient care services, prioritizing the comfort and dignity of our patients.
  • Maintain accurate and thorough documentation of patient care and services provided.

Requirements:

  • High school diploma or equivalent, with completion of a Board of Nursing-approved training program and current listing with the Division of Facility Services Registry.
  • At least one year of patient care experience in an acute care or custodial setting.
  • Documented proof of 75 certified education hours.
  • Ability to work independently, providing effective healthcare services to homebound patients.
  • Working knowledge of safe and appropriate personal care practices, as well as company policies and procedures.
  • Minimum of 12 hours of continuing education annually.
