At LibertyHealth System, we provide compassionate care to patients and families facing terminal illness. Our hospice team delivers state-of-the-art care and pain management services, emphasizing strength, dignity, and compassion. We are seeking an experienced Registered Nurse to join our team in providing high-quality care to our patients.
Job Responsibilities:- Apply nursing theories and concepts to provide patient care.
- Coordinate physical care of patients with families, physicians, and the hospice team.
- Communicate effectively with patients, families, and agency personnel.
- Participate in interdisciplinary team meetings and quality reviews.
- Current RN licensure in the state of practice.
- Two years of clinical nursing experience and at least one year of hospice nursing experience preferred.
- Knowledge of home health and hospice care programs and the nurse's role in patient care.
- 12 hours of continuing education annually.
- CPR certification.
- Ability to accept travel assignments and weekend/evening call duties.
