Job Description

Job Summary

We are seeking a highly organized and communicative Residential Service Coordinator to oversee a home with up to four individuals with intellectual and developmental disabilities. As a key member of our team, you will provide coordination and support to ensure the health, safety, and well-being of our clients.

Key Responsibilities

  1. Participate in interdisciplinary team meetings to develop and implement individualized support plans.
  2. Review and analyze reports and documentation to identify areas for improvement and provide guidance to support staff.
  3. Ensure medical needs are met, including scheduling appointments, managing medications, and communicating with healthcare professionals.
  4. Advocate for and support individuals with intellectual and developmental disabilities in accessing community resources and services.
  5. Act as a positive role model and provide emotional support to individuals and their families.
  6. Assist with daily living tasks, such as hygiene, grooming, and meal preparation.
  7. Ensure the home environment is safe and welcoming.
  8. Provide transportation and access to community resources and services.
  9. Coordinate staffing and ensure adequate coverage to maintain health and safety.
  10. Provide training and mentorship to support staff to ensure success in their roles.
  11. Evaluate employee performance and provide feedback and coaching.
  12. Complete required training and adhere to agency policies and procedures.
  13. Maintain regular communication with healthcare professionals, program directors, and HR.
  14. Participate in internal and external audits and surveys.

Additional Responsibilities

  • Manage day-to-day operations, including petty cash and supplies.
  • Conduct regular team meetings and provide feedback and coaching.
  • Verify medication management and safety protocols.
  • Ensure nutritional recommendations are met through meal planning and grocery shopping.
  • Teach and support staff in Therap and GER processes.
  • Client funds reconciliation and ledger management.
  • Attend meetings and provide oversight for secondary locations as needed.

Requirements

  • High school diploma or equivalent.
  • Current and valid driver's license.
  • Safe and insured vehicle.
  • Minimum 1 year of human service management experience or completion of the HeartWell Management Training Program.
