Care Manager RN

We are seeking a skilled Registered Nurse PRN Care Manager to join our team at AdventHealth. As a Care Manager, you will play a vital role in ensuring the seamless transition of care for our patients, from hospital to home.
Key Responsibilities- Conduct thorough patient evaluations to identify post-hospital needs and develop effective transition of care plans.
- Collaborate with the interdisciplinary team to ensure efficient and cost-effective care, while promoting patient satisfaction and safety.
- Communicate daily with the team during multidisciplinary rounds to review patient progress and discharge plans.
- Identify and address potential barriers to care, including readmission risks and medical necessity.
- Provide education and support to patients, families, and healthcare teams on issues related to care coordination and transitions.
- Current RN licensure and certification in a relevant specialty.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Strong analytical and problem-solving skills.
