  • Coordinate care needs with community providers through an effective case management process, ensuring seamless care transitions.
  • Balance service demands with supporting resident independence, promoting autonomy and dignity.
  • Recognize and assist in the prevention of elder abuse, neglect, and exploitation, reporting incidents to appropriate authorities as required by state regulations.
