Emergency Department Patient Care Coordinator

Cedar Park Regional Medical Center is seeking a highly skilled and compassionate Emergency Department Patient Navigator to join our team. As a Patient Navigator, you will play a vital role in ensuring that patients receive the highest level of care and support during their stay in the Emergency Department.
Job Summary:The Patient Navigator will assist unassigned patients presenting to the Emergency Department with selecting a Primary Care Provider (PCP) from one of our affiliated clinics. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Key Responsibilities:- Utilize our electronic medical record system to screen patients at bedside in the ED and IP settings
- Track patient visits daily on a spreadsheet and review for possible scheduling of follow-up appointments
- Make phone calls to patients seen through the Emergency Department during off-hours who meet scheduling criteria
- Schedule follow-up appointments via our electronic medical record system and provide patients with collateral for their appointment, including a brochure with the address, phone number, and location map for the physician office
- Make reminder calls to patients the day prior to their scheduled physician office appointment
- Contact patients one day post-physician office visit to ensure their satisfaction with the visit and confirm that their needs were met
- Attend daily Case Management meetings to review progress on patients who have been admitted to the hospital on either an outpatient or inpatient basis
- Coordinate scheduling for follow-up exams or tests ordered by the ED physician, as well as schedule procedures through our Central Scheduling Department
- Maintain a log of all scheduled procedures and report daily to the PAS, Physician Practice, and CFO/CEO
- Complete other tasks as assigned, including DCA for patients without PCPs, Peripheral Imaging findings/follow-up scheduling, FSED and other transfers, follow-ups, discharge summaries, and progress notes
- Answer all calls in a professional manner and greet visitors and staff members with respect
- Minimum of a High School Diploma or GED required
- Two (2) + years of healthcare/medical office experience strongly desired
- High level of customer service experience required
- Working knowledge of third-party payers and insurance verification and authorization procedures
- Medical terminology strongly preferred
- Demonstrates knowledge of governmental regulations regarding the confidentiality of patient information
- Strong computer and internet skills required
