Job SummaryWe are seeking a skilled and compassionate Registered Nurse to join our team as a Palliative Care Specialist. In this role, you will provide high-quality care to patients with life-limiting illnesses, working closely with our interdisciplinary team to develop and implement personalized care plans.
Key Responsibilities- Conduct thorough nursing assessments to identify patient needs and develop effective care plans
- Provide direct patient care, including medication management and education
- Collaborate with healthcare providers to coordinate care and ensure seamless transitions
- Document patient progress and outcomes in a timely and accurate manner
- Participate in ongoing education and professional development to stay current with best practices in palliative care
- Current, active RN licensure in the state of North Carolina
- Minimum 2 years of experience in direct patient care, preferably in hospice or palliative care
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
- Computer proficiency and ability to navigate electronic medical records
