About the Role

Felton Institute is seeking a highly skilled Clinical Case Manager to join our Older Adult Service Team (OAST) in Alameda County. As a key member of our multidisciplinary team, you will provide comprehensive case management services to older adults with severe mental illness and co-occurring disorders.

Key Responsibilities
  • Conduct intakes and assessments for older adults with severe mental illness, developing individualized treatment plans to address their complex behavioral health and medical needs.
  • Provide case management services, including evaluations, treatment plan development, and assistance with accessing housing, financial, medical, and other needed services.
  • Collaborate with an interdisciplinary team of clinical case managers, psychiatric physician assistants, psychiatrists, and community/peer specialists to provide intensive services to older adults.
  • Perform crisis intervention and management to prevent hospitalization, maintaining accurate and detailed documentation in accordance with mandated standards.
  • Participate in team meetings and case conferences, presenting cases and discussing clinical issues to ensure seamless care coordination.
  • Commit to ongoing professional development in geriatric mental health services, participating in trainings offered by Felton Institute or outside providers.
Requirements
  • A minimum of two years of case management and mental health treatment experience for at-risk older adults, including co-occurring disorders.
  • Masters degree or higher in Social Work, Counseling, or Psychology, with active registration with the BBS (AMFT, APCC, ASW, LMFT, LPCC, LCSW) highly preferred.
  • Comprehensive range of mental health and substance abuse counseling, assessment, and crisis intervention skills.
  • Ability to work cooperatively on a multi-disciplinary team and with community partners and collaborators.
  • Knowledge of DSM V-TR and experience with Medi-Cal billing requirements.
  • Valid CA Driver's License and vehicle.
